[step-manufacturing] Updated AP-238 ready for review!

Hi Everyone, 

I have posted an updated AP-238 document for review prior to Bath.
This is not quite ready for DIS, but has many new features, such as
support for the Turning Model (Part 12), expanded ARM definitions, CD
ballot resolutions, and a new "Fundamental concepts and assumptions"
section.   You can find it at:

The "Fundamental concepts" section addresses the relationship with ISO
14649 and clarifies many subtle things discovered in the implementation
tests.  It's not too long but it sure took a lot of hard work to write!
